Who We Support: Target Population
We specialize in supporting:
- Adults aged 18 and older with developmental disabilities
- Individuals who have chosen to live at home with family
- Clients receiving services through California regional centers (Lanterman Act)
Our approach is particularly for:
- Young adults transitioning out of school-based programs
- Families of adults with complex needs requiring system navigation expertise
- Aging parents looking for future planning and support solutions
We don’t just accommodate diversity — we design for it. Whether a family speaks Spanish, Tagalog, or ASL, whether they are newly navigating the system or burned out from years in it — we meet them where they are and move forward together.
Entrance Criteria
To be eligible for enrollment in Rise Above & Beyond Emotions, Inc.‘s Coordinated Family Support program, individuals must meet the following criteria:
- Be an active client of a California Regional Center.
- Be 18 years of age or older.
- Currently reside in the family home with a parent, sibling, or relative caregiver.
- Have an Individual Program Plan (IPP) goal that specifically supports remaining at home with coordinated supports.
- Not be currently enrolled in California’s Self-Determination Program, as per Department of Developmental Services (DDS) guidelines for CFS.
- Both the individual and their family must provide consent for services.
We particularly prioritize individuals and families who:
- Are in the process of transitioning from school-based services to adult services.
- Are managing multiple and complex service providers and require assistance with coordination.
- Face challenges due to cultural or language barriers when navigating the support system.
- Are experiencing gaps in care as a result of caregiver burnout, illness, or social isolation.
Rise Above & Beyond Emotions, Inc. has significant experience in supporting families from under-resourced communities and those who traditional support systems have historically underserved.
Our Expected Client Outcomes
At Rise Above & Beyond Emotions, Inc., our Coordinated Family Support (CFS) program is intentionally designed to achieve the following tangible and qualitative results for individuals, their families, and the broader support system:
Individual Outcomes:
- Individuals will engage more frequently in activities within their local community.
- They will develop stronger skills in self-advocacy and making their own choices.
- Their abilities to manage daily routines and live more independently will improve.
- They will have better access to necessary health and behavioral health services.
- A more reliable and personalized network of support, both formal and informal, will be established around them.
Family Outcomes:
- Caregivers will experience less stress and reduced burnout.
- Families will feel more confident in their ability to navigate complex support systems.
- Clear and effective backup and emergency care plans will be in place.
- The alignment between the family’s culture and the services provided will be enhanced.
- Collaboration with regional centers and other service providers will be stronger.
Systemic Outcomes:
- Generic, community-based services will be utilized more efficiently.
- There will be fewer disruptions in services or emergency placement situations.
- The coordination between day programs, medical care, and in-home support will be improved, reducing gaps.
- High levels of satisfaction and engagement will be seen from families, particularly within underserved communities.
Rise Above & Beyond Emotions, Inc. monitors progress through detailed service notes, regular quarterly reports, and feedback gathered from family surveys. However, our most crucial method is active listening – we prioritize understanding the individual journey and celebrating every step of growth alongside our clients and their families.
Exit Criteria
Transitioning from Rise Above & Beyond Emotions Inc. Support
Our CFS services may conclude when:
- The individual moves out of the family home.
- The family or individual chooses to discontinue support.
- The individual enrolls in the Self-Determination Program (SDP).
- The regional center determines our service is no longer appropriate.
- The family and our team agree goals are met, and independent navigation is possible.
- Repeated disengagement from support occurs despite our outreach.
When exit is needed, Rise Above & Beyond Emotions Inc. will:
- Provide a written transition plan.
- Offer referrals to other appropriate services.
- Notify the Service Coordinator and family in writing.
- Hold a closure meeting to celebrate achievements (if desired).
For us, transitioning support is a step towards greater autonomy, a growth milestone, rather than a setback. Our ultimate goal remains empowering self-reliance, even when our direct involvement concludes.
