Our Service Delivery Model

Intake & Assessment Process

At Rise Above & Beyond Emotions Inc., we believe in a thoughtful and unhurried intake process, prioritizing building a strong connection with your family from the start.

Our step-by-step process includes:

  1. Receiving the WRC Referral and making a Welcome Call to you within two business days.
  2. Conducting an introductory visit in your home with the assigned CFS Specialist and their Supervisor.
  3. Completing a thorough assessment of your needs using a visual tool that explores different areas of life.
  4. Engaging in culturally sensitive interviews to understand your goals, needs, and family dynamics.
  5. Identifying and addressing any urgent, time-sensitive support needs immediately.
  6. Developing an Individualized Service Plan draft with clear and measurable outcomes.
  7. Reviewing the plan with your family and commencing services upon agreement.

We view this initial phase not just as gathering information but as a crucial opportunity to deeply understand the values, hopes, and concerns that will guide our collaborative work.

Development & Monitoring of Service Plans

Service plans are dynamic tools, regularly updated to capture evolving goals, available resources, and changes within the family environment.

    •  Quarterly triad meetings (Family + RC + CFS)
    • Regular email summaries and check-ins to maintain alignment
    • Joint participation in IPP meetings
    • Wraparound team meetings when multiple services are involved

Quality Assurance

  • Quarterly service reviews evaluating quality metrics
